FAQ for your first virtual visit

As do most practices our office does have routine office intake questionnaires and our doctors go the extra mile by taking the time to learn about each patient's communication style before providing care to ensure that patient instructions are delivered in the best way possible. This is done via a 8 question quiz that takes approximately 3 minutes to complete and you will be requested to complete this at the beginning of your virtual visit.

Our office does not cancel appointments because of an inability to complete this quiz.

Our office does require before providing service to anyone that certain standards are upheld: financial consents are signed, health history is provided and if using insurance copayment must be made at the time of service.

We post these requirements both in writing and these requirements are visible in our waiting room. We are unable to provide care to any person who is not willing to abide by the requirement to sign all consents and to make their payment at the time of service as a general policy.
