**We do not accept Medicaid, Medicaid HMO's or Medicare**

If you don't see the name of your insurance on this list but you do see our office's name (Crossroads ObGyn and Wellness) or the physician's name (Kourtney Sims, MD) as an in-network provider listed on your insurance plan then we likely are in your network and you can go ahead and schedule your appointment here. 

We do complete a manual phone verification on all new patients and for established patients who have a change in their insurance so if we do happen to not be in your network we will notify you.
