Payment Estimate

Prior to your office visit our office makes every attempt to verify your insurance and benefits.  If based on this pre-verification it is determined that you may have a patient responsible amount under your copay, co insurance, and or deductible then our office will send you a payment estimate. This payment estimate will reflect the amount of patient responsibility that is due at your time of service. This estimate is sent directly to your email address and not to any patient portal.

Please be advised that this is an estimate only. Once your claim is filed to your insurance and your insurance adjudicates that claim, if there is any difference in the amount of patient responsibility you will be updated via your explanation of benefits that is sent from your insurance.

If during the pre visit verification it is determined that your expected patient responsible amount is $0.00 then you will not be sent a payment estimate.

Instead we recommend that you submit a help ticket via our support desk online which you may access at crossroadsobgyn.com.
