Medical Record Requests Fees

 All requests must be received in writing on an approved form.  We encourage you to actively download copies of your medical records from your portal including visit notes and results as you review them and after your visit as if your care is terminated with our office your portal access will terminate immediately and you will have to request your records formally. We cannot process verbal requests only for medical records. Charges and rates are set by the State of Texas and payment must be remitted prior to transfer of records even when being requested by another healthcare provider unless there is a medical emergency (as defined by a request from a hospital after you have been admitted or an emergency room).  These fees typically start at $25.
