How do I upload or add my insurance card to your system?

Please see instructions below for instructions on how to add your insurance card to our system.

 

Adding a copy of insurance card 

1. Login to your patient portal. Make sure that you are using the full site version if you are using a mobile device by clicking on the full site link at the bottom of your mobile browser page.

2. Scroll down on your portal homepage to the Messages section and click on Send a Message

3. In the dropdown box under provider select Kourtney Sims M.D. then under the drop down box for subject select Insurance. In the message section please enter any information that you want us to know about your insurance policy such as if your policy is under a spouse or a parent. Then please attach a copy of the front and the back of your insurance card on the form.  

4.  Click on the send message blue button and that's it!
