How Do I Add My Test Results, Imaging or Other Documents to My Athena portal?

Please see instructions below for instructions on how to add your tests results, imaging, supplements or any other documents for your medical chart to our system.

1. Login to your patient portal. Make sure that you are using the full site version if you are using a mobile device by clicking on the full site link at the bottom of your mobile browser page.

2. Scroll down on your portal homepage to the Messages section and click on Send a Message

3. In the dropdown box under provider select Kourtney Sims M.D. then under the drop down box for subject select Insurance. In the message section please enter any information that you want us to know about the documents you have uploaded. Then please attach the documents that you want to send us.

4.  Click on the send message blue button and that's it!
