How Do I Know I Am Checked In?

You will know when your check is complete as you will receive a confirmation email with the following subject line:   "Your clinical history forms have been received for your appointment" and it will also include the date of your appointment that we received the check in for. It will also contain the following picture:

If you do not or have not received an email with the subject line or picture as shown above then we DO NOT have your check in.
