Missing or need care summary or notes from my visit
We understand that many patients review their care summary and notes from their visit with the doctor to ensure that they are following their care plan. Typically care summaries are published to the patient portal within two business days of checkout.
If it has been more than two business days since you have completed your visit check out and you still do not see your visit summary on your patient portal then we may need to republish your care summary to your patient portal. To request a new copy of your visit notes or care summary please submit a help ticket through our support desk at crossroadsobgyn.com.
Once you have submitted your request to republish your care summary to your patient portal please allow 48 hours for office to process your requests and follow up on your request.
