How To Terminate Care with Office
We are honored for the time that you allowed us to participate in your healthcare journey.

Please note that any balances that are on your account that are >30 days old per your financial policy will be immediately forwarded to collections. Any accounts <30 days old that are not remitted to the office (you may pay online or send your balance to the account on your patient statement) within 30 days of statement will be referred to collection at 30 days past due. Therefore we do recommend clearing your account before you submit your termination request. If you have questions for the billing department you may submit those @ help@houstonsobgyn.zendesk.com.
If you are leaving the practice we would like to kindly ask you for your feedback on your healthcare experience to assist in improving our office practices. As much as we strive to be we know that we are human and therefore not perfect so we want to make sure that we take the time to ask you for feedback in regards to our services. Please make sure to provide this information on your termination request form or on the exit interview form that may be sent after we receive your termination request.
